I agree that I will make no monetary or other claim against Formless Beauty, LLC for the use of my digital photographs, measurements, written testimonial and audio/video testimonial. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used except as provided herein. I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or my testimonial appears. I represent that I am at least 18 years of age, have read and understand the foregoing, and am competent to execute this agreement. I hereby hold harmless and release Formless Beauty, LLC from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization I have read the authorization and release information and give my consent for the use as indicated above.
